Have Medicaid
Live in Indiana
Need help with day-to-day activities
Able to direct your own care/services (if not, have an individual representative to appoint)
60 and over and qualify for the Pathways MLTSS Waiver, or
59 and under and qualify for the Health & Wellness (formerly Aged & Disabled) Waiver
Medicare is NOT enough; You must have Medicaid to qualify!
How it Works for the Patient

Someone who is at least 18 years of age
Someone who eligible for work in the United States
Someone who is not excluded from the Medicaid program
The Caregiver can be either a family member or a non-family member residing with the Participant in the private home of either the Participant or the Caregiver. Additionally, in some Medicaid programs we offer, the Caregiver is not required to live with the Participant.
Receive care from a loved one who is compensated through an Indiana Medicaid Waiver program!
Family members and friends can now be paid to provide care to a loved one who qualifies. The Structured Family Caregiving (SFC) and Attendant Care (ATTC) programs, part of Indiana’s Medicaid Waiver options, allow caregivers to receive payment for supporting a loved one in need, provided all eligibility requirements are met.
